1.0 Purpose
To establish standard procedures for immediate verification, verbal communication, read-back confirmation, and documentation of critical (panic) values and unusual laboratory results, and to prescribe the protocol for identifying, documenting, and reporting notifiable infectious diseases detectable on TCH's testing menu in compliance with Republic Act No. 11332 while strictly preserving patient confidentiality.
2.0 Scope
This policy applies to all Medical Technologists, Laboratory Aides, and clinical/nursing staff involved in examining, verifying, communicating, or receiving critical values, unusual findings, and notifiable disease results within TCH's scope of service. HIV testing is not performed at TCH; any referred HIV results are handled under statutory confidentiality requirements.
3.0 Definitions
- Critical (panic) laboratory value — a result representing an acutely abnormal pathophysiological state that indicates an immediate, life-threatening danger to the patient and requires prompt clinical intervention.
- Read-back — the mandatory patient-safety communication process where the receiving clinician or nurse repeats the communicated patient identifiers, test name, and critical value back to the notifying Medical Technologist, who verbally confirms accuracy before terminating the call.
- Unusual result — a laboratory finding that is unexpected or markedly inconsistent with the patient's prior results (delta check failure) or clinical presentation, requiring analytical verification and review before final authorization.
- Notifiable disease / health event of public health concern — any condition or disease required to be reported to the Department of Health (DOH) or local health authorities pursuant to Republic Act No. 11332 and its Implementing Rules and Regulations.
- Double-testing confirmation — TCH's verified standard operating practice for a reactive or positive serological screening result for Dengue, Hepatitis B (HBsAg), or Syphilis (RPR), wherein the examination is repeated once on the original sample to confirm analytical reproducibility before communicating the finding to the clinician.

5.0 Policy / Procedure
5.1 Policy Statement
All verified critical values shall be communicated immediately to the responsible healthcare provider — within 15 minutes of verification — by direct telephone notification to the ordering physician, duty physician, or designated nurse-in-charge. Notifications shall never be left with non-clinical support personnel. Critical values received from a referral laboratory (see Referral Laboratory Policy) shall be handled with the same immediate priority.
All unusual results and reportable infectious disease findings shall be investigated, verified, documented, and reported in full compliance with patient privacy safeguards.
The approved TCH laboratory critical value thresholds requiring immediate clinical escalation are:
| Examination / Analyte | Conventional Units | SI Units | Critical Low Threshold | Critical High Threshold |
|---|---|---|---|---|
| Hemoglobin (Adult) | g/dL | g/L | < 6.5 g/dL (< 65 g/L) | > 20.0 g/dL (> 200 g/L) |
| Hematocrit (Adult) | % | Volume fraction | < 20.0% (< 0.20) | > 60.0% (> 0.60) |
| White Blood Cell Count | x 10^3/uL | x 10^9/L | < 2.0 x 10^3/uL (< 2.0 x 10^9/L) | > 30.0 x 10^3/uL (> 30.0 x 10^9/L) |
| Platelet Count | x 10^3/uL | x 10^9/L | < 50 x 10^3/uL (< 50 x 10^9/L) | > 1,000 x 10^3/uL (> 1,000 x 10^9/L) |
| Peripheral Blood Smear | Morphology | Morphology | Initial detection of blasts or intracellular micro-organisms | Acute leukemic presentation |
| Glucose (Adult / Pediatric) | mg/dL | mmol/L | < 45 mg/dL (< 2.5 mmol/L) | > 450 mg/dL (> 25.0 mmol/L) |
| Glucose (Neonatal) | mg/dL | mmol/L | < 30 mg/dL (< 1.7 mmol/L) | > 300 mg/dL (> 16.7 mmol/L) |
| Potassium (K+) | mmol/L | mmol/L | < 2.8 mmol/L | > 6.2 mmol/L |
| Sodium (Na+) | mmol/L | mmol/L | < 120 mmol/L | > 160 mmol/L |
| Creatinine | mg/dL | umol/L | None | > 5.0 mg/dL (> 442 umol/L) new-onset |
5.2 Verification Before Notification
Before initiating critical value communication, the Medical Technologist shall rapidly verify:
- Patient identity and specimen label concordance across the collection tube, requisition, LIS entry, and instrument output.
- Sample integrity (checking for microclots, gross hemolysis, lipemia, or insufficient volume).
- Instrument performance and internal quality control (IQC) acceptability on the active analytical run.
- Immediate re-testing from the primary tube where clinically or analytically indicated.
5.3 Notification Script and Read-Back
The Medical Technologist shall execute the standardized verbal read-back script: *"Hello, this is [Name/ID] from the Clinical Laboratory. I have a critical laboratory result for your patient. Please record: Patient [Full Name], Hospital ID/MRN [Number], located at [Ward/Unit]. Test [Test Name], Critical Result [Value and Units].
Please read this information back to me."* The Medical Technologist shall listen carefully to the read-back and conclude only after confirming accuracy ("Confirmed, thank you") or immediately correcting any discrepancies.
5.4 Escalation Notification Sequence
When communicating critical values, staff shall follow this strict notification hierarchy:
- Primary Contact: Call the requesting or attending physician's direct telephone contact.
- First Escalation: If the attending physician is unavailable, immediately contact TCH's 24-hour on-duty hospital physician.
- Second Escalation: If the duty doctor is unreachable, notify the nurse-in-charge at the patient's care unit (Inpatient Nurse Station operates 24/7; Emergency Room handles after-hours cases, per Laboratory Reporting Policy).
- Administrative Escalation: If all three contacts fail, escalate immediately to the Chief Medical Technologist / Laboratory Quality Manager (see Figure 1).

5.5 Documentation Requirements
Every critical value and notifiable disease notification event shall be recorded immediately in F-QM-POST-002-A-V0 (Critical Value & Notifiable Disease Notification Log), documenting:
- Date and exact time of notification.
- Patient full name and unique identification number (MRN).
- Test name, critical or reactive result, and units.
- Name and title of the person notified.
- Confirmation of successful read-back.
- Name and signature of the notifying Medical Technologist.
5.6 Patient Identification Safeguards
Patient identification shall strictly follow Specimen Collection Policy Section 5.3, utilizing at least two independent identifiers (full patient name and hospital MRN). Room, bed, or station numbers shall never be used as patient identifiers.
5.7 Unusual Laboratory Results
Unusual, non-critical results (such as delta check failures exceeding plausible biological variation or unexpected microscopic elements) shall be investigated before release:
- The Medical Technologist shall repeat the test from the primary container and verify quality control.
- The finding shall be reviewed with the Chief Medical Technologist / Laboratory Quality Manager alongside the patient's recent clinical and testing history.
- If the finding is clinically unexpected or requires diagnostic clarification, the Chief Medical Technologist or Pathologist shall consult the ordering physician, documenting findings in F-QM-POST-002-A-V0.
5.8 Notifiable Infectious Diseases
5.8.1 Scope and HIV Contingency
TCH Clinical Laboratory does not perform in-house HIV testing. If an HIV examination is clinically indicated, specimens are referred to an accredited referral laboratory per Referral Laboratory Policy. Any referral HIV result returned to TCH shall be handled under strict confidentiality per Republic Act No. 11166 and Confidentiality Policy.
5.8.2 Menu Scope and Double-Testing Confirmation
TCH performs rapid screening tests for notifiable infectious diseases within its licensed capability:
- Dengue (Dengue Duo NS1/IgM/IgG).
- Hepatitis B (HBsAg).
- Syphilis (RPR).
- Helicobacter pylori.
To ensure high diagnostic reliability, any reactive or positive result for Dengue, HBsAg, or RPR shall undergo immediate double-testing confirmation (re-tested once on the same specimen using a new test device) before release. H. pylori is reported from a single test run.
5.8.3 Notification and Reporting Workflow
- The performing Medical Technologist shall communicate confirmed reactive findings to the attending physician per Laboratory Reporting Policy, explicitly noting that duplicate testing was performed.
- Notification details shall be logged in F-QM-POST-002-A-V0.
- The Laboratory shall notify the TCH Infection Prevention and Control (IPC) focal person and report applicable notifiable cases to the Department of Health (DOH) in accordance with Republic Act No. 11332 and designated surveillance channels.
5.8.4 Retention of Reactive Specimens
All reactive or positive specimens for notifiable infectious diseases (Dengue, Hepatitis B) shall be stored under monitored refrigeration or freezing for a minimum of six (6) months per Laboratory Retention Schedule Policy to allow confirmatory referral or epidemiological verification.
