1.0 Purpose
To establish an operational business continuity and disaster recovery plan for the Tarangnan Community Hospital (TCH) Clinical Laboratory during computer hardware failure, electronic spreadsheet corruption, local network interruptions, or electrical power outages. This policy ensures uninterrupted emergency and routine testing workflows through a standardized manual alternate process, establishes recovery protocols using off-site cloud backups, and ensures full compliance with DOH licensing requirements and ISO 15189:2022 Clauses 7.6.4 and 7.8.
2.0 Scope
This plan applies to all Medical Technologists and Laboratory Aides across all sections of the TCH Clinical Laboratory. It governs manual specimen accessioning, manual analytical worksheets, handwritten reporting, emergency telephone communication of STAT and critical results, post-restoration data entry, and physical disaster equipment recovery.
3.0 Definitions
- Downtime — any planned or unexpected period where laboratory computers, printers, or local power supplies are unavailable for routine electronic data entry and report printing.
- Disaster — an unforeseen catastrophic event (e.g., severe flood, typhoon, electrical fire, structural collapse) that destroys laboratory IT hardware or compromises facility integrity.
- Manual Alternate Workflow — the approved fallback procedure utilizing paper accession registers, manual bench worksheets, and pre-printed laboratory report forms.
- Pending-Work Log — the temporary operational log activated during downtime to track all specimens accessioned and tested manually until restored to the Lab-LIS.
4.0 Responsibility
4.1 Hospital Director
- Ensures the hospital emergency power generator automatically supplies backup electrical power to the clinical laboratory during main utility failures.
- Approves emergency procurement or hardware replacement in the event of catastrophic IT equipment destruction.
4.2 Pathologist
- Advises on clinical testing prioritization during extended downtime and confirms technical data integrity during disaster recovery restoration.
- Holds secondary authorized custody of cloud backup credentials to facilitate system restoration if local files are destroyed.
4.3 Chief Medical Technologist / Laboratory Quality Manager
- Declares the official activation and termination of laboratory downtime procedures.
- Oversees post-restoration data reconciliation, coordinates hardware replacement with municipal IT personnel, and conducts annual downtime simulation drills.
- Performs quarterly test restores of the cloud-synchronized Lab-LIS backup from Google Drive.
4.4 Medical Technologists
- Execute the manual alternate workflow immediately upon computer or power interruption, prioritizing STAT and emergency examinations.
- Maintain accurate entries in the Downtime Pending-Work Log (F-QM-IT-002-A-V0), issue authorized handwritten reports, and telephone critical values with documented read-back.
- Back-encode all completed manual worksheets into the electronic Lab-LIS once computer operations resume.
4.5 Laboratory Aides
- Assist in logging manual accession numbers, delivering paper reports to clinical wards, and notifying nursing units of downtime status under technologist direction.
- Do not perform, calculate, or interpret laboratory tests or verify clinical reports.
5.0 Policy / Procedure
5.1 Downtime Declaration and Activation
- Trigger Criteria: Whenever the laboratory computer cannot be booted, spreadsheet files cannot be opened, or power fails beyond the battery capacity of the Uninterruptible Power Supply (UPS), the technologist on duty shall declare system downtime.
- Immediate Notifications: The on-duty technologist shall immediately inform the Chief Medical Technologist / Laboratory Quality Manager and notify inpatient nursing stations and the emergency room that manual reporting is active (see Figure 1).

5.2 Pre-Analytical Manual Workflow
- Specimen Acceptance: Samples shall be received using standard paper requisition slips.
- Manual Accessioning: Technologists shall assign manual accession numbers sequentially in the Laboratory Master Logbook, recording: - Date and time received; - Complete patient name, age, sex, and hospital/ward number; - Requested diagnostic examinations; - Clinical priority (STAT vs. Routine).
- Specimen Labeling: Primary specimen tubes must be legibly labeled by hand with at least two unique patient identifiers (full name and hospital number) using waterproof ink.
5.3 Analytical Operations and STAT Prioritization
- Analytical Continuity: Primary analyzers (HORIBA Pentra XLR, wet chemistry analyzer, POCT meters) shall continue running on emergency generator power. If analyzers are inoperative due to power loss, manual backup testing (e.g., manual microscopy, manual slide testing) shall proceed where validated.
- STAT Processing: STAT requests, emergency room orders, and delivery room specimens shall be processed with top priority in accordance with Turnaround Time and Stat Testing Policy.
- Worksheet Tracking: Technologists shall record all raw instrument readouts, calibration checks, and daily internal QC directly on designated bench worksheets.
5.4 Post-Analytical Reporting and Critical Values
- Handwritten Reports: Diagnostic results shall be legibly transcribed onto standard pre-printed, multipart carbonless laboratory result slips.
- Critical Value Notification: Critical results must be phoned out immediately to the attending physician or nurse on duty with mandatory read-back verification and logged per Reporting Critical Values Policy.
- Pending-Work Log: Every manual report issued shall be logged immediately on the LIS and Power Downtime Contingency Log (F-QM-IT-002-A-V0) to prevent omitted, delayed, or duplicated records.
5.5 Post-Restoration Data Reconciliation
- System Resumption: Once power or computer operation is restored, the Chief MT / LQM shall verify system stability before initiating back-encoding.
- Re-encoding Workflow: All tests logged on F-QM-IT-002-A-V0 shall be entered systematically into the electronic Lab-LIS from the paper worksheets.
- Transcription Verification: A second technologist or the Chief MT shall cross-check electronic entries against paper originals to verify zero transcription errors before closing out the downtime event.
5.6 Disaster Recovery Protocols
- Post-Disaster Assessment: Following a catastrophic physical disaster (fire, storm damage, severe water ingress), the Chief MT / LQM and Pathologist shall assess workstation usability, hardware integrity, and power stability.
- Cloud Restoration: If the physical workstation or internal hard drive is destroyed: - Municipal IT support shall supply a replacement computer running compatible Windows and Microsoft Excel; - The Chief MT or Pathologist shall access the dedicated laboratory Google Drive cloud account; - The latest synchronized version of the Lab-LIS templates, historical patient records, and Levey-Jennings QC spreadsheets shall be downloaded; - Worksheets generated during the disaster interval shall be re-encoded to establish a complete electronic audit trail.
- Recovery Verification: The restored system shall be verified using five (5) control datasets to confirm calculation accuracy before resuming live reporting.
5.7 Annual Continuity Drills and Policy Review
- Testing Cadence: The laboratory shall conduct a simulated downtime and manual alternate workflow test at least annually.
- Restore Drills: A mock disaster restore test from the off-site Google Drive repository onto a secondary device shall be executed and documented annually on F-QM-IT-001-A-V0.
- Annual Review: This plan shall be formally reviewed annually by the Chief Medical Technologist / Laboratory Quality Manager and Pathologist to ensure ongoing alignment with hospital emergency management protocols.
