1.0 Purpose
To establish a unified operational policy for maintaining up-to-date and confidential personnel records and managing staff occupational health in the Tarangnan Community Hospital (TCH) Clinical Laboratory. This policy ensures strict compliance with DOH licensing requirements, ISO 15189 standards, the Data Privacy Act, and occupational health mandates by maintaining complete 201 dossiers, tracking mandatory immunizations, and safeguarding staff welfare.
2.0 Scope
This policy applies to all personnel engaged in the TCH Clinical Laboratory — Pathologist, Chief Medical Technologist / Laboratory Quality Manager, Medical Technologists, Laboratory Aides, and temporary/Job Order staff. It encompasses the two-layer personnel record architecture, annual credential currency reviews, baseline and periodic health assessments, Hepatitis B and Influenza vaccinations, occupational exposure follow-ups, and medical return-to-work clearances.
3.0 Definitions
- Layer A (Institutional 201 File) — the official human resources dossier maintained by the hospital HRMO containing civil service appointments, contracts, institutional evaluations, and payroll records.
- Layer B (Laboratory QMS Personnel File) — the departmentally maintained quality record containing copies of diplomas, PRC licenses, orientation logs, competency authorizations, CE/CPD credits, and health certificates.
- Occupational Health Surveillance — systematic monitoring of laboratory staff health status, including pre-placement screening, annual physical examinations, and immunization records.
- Occupational Exposure — percutaneous injury (needlestick, cut), mucous membrane contact, or non-intact skin contact with blood, body fluids, or other potentially infectious materials.
- Work Restriction — temporary clinical limitation or modification of duties imposed on an employee due to communicable infection, acute illness, or post-exposure protocol to protect patients and staff.
4.0 Responsibility
4.1 Hospital Director
- Oversees institutional compliance with the Data Privacy Act (RA 10173) and Occupational Safety and Health Standards (RA 11058).
- Authorizes hospital medical services to provide free Hepatitis B and Influenza vaccines, pre-employment screenings, and emergency post-exposure prophylaxis for laboratory workers.
4.2 Pathologist
- Advises on technical aspects of staff health surveillance and provides technical consultation during occupational exposure investigations.
- Ensures their own professional credentials (specialty board certificate, PRC license, and DOH registration) are current and filed in the laboratory.
4.3 Chief Medical Technologist / Laboratory Quality Manager
- Manages all Layer B Laboratory Personnel Files (F-QM-HR-004-A-V0) and conducts mandatory annual currency audits of professional licenses and health certificates.
- Maintains the official List of Personnel with Designations (F-QM-HR-004-B-V0) and submits required workforce data to the HRMO and DOH NDHRHIS.
- Enforces work restrictions for personnel with communicable illnesses, monitors post-exposure follow-up, and maintains strict confidentiality of all medical records.
4.4 Medical Technologists
- Provide authenticated copies of PRC licenses, board certificates, diplomas, CPD credit logs, and medical certificates upon appointment and renewal.
- Participate in mandatory vaccination programs or execute formal declination forms, and immediately report all needlestick injuries and bloodborne exposures.
4.5 Laboratory Aides
- Provide required educational and health clearance documentation upon engagement, and submit to annual medical physical examinations.
- Comply with mandatory immunization and personal protection policies; immediately report any accidental cuts, chemical splashes, or waste handling injuries.
5.0 Policy / Procedure
5.1 Two-Layer Personnel Record System
To eliminate administrative duplication while satisfying DOH licensing and ISO 15189 requirements, TCH operates a distinct two-layer personnel record architecture:
- Layer A (Hospital HRMO Dossier): Official repository owned and controlled by the municipal HRMO, retaining original employment appointments, Job Order contracts, Service Cards, and institutional leaves.
- Layer B (Laboratory QMS Personnel File): On-site active operational file owned by the Chief Medical Technologist / Laboratory Quality Manager, containing technical qualifications, training records, competency authorizations, and occupational health clearances.
- Regulatory Audit Interface: The laboratory maintains the technical records on-site to facilitate immediate verification during DOH regulatory inspections and accreditation audits without requesting HRMO records.
5.2 Required Personnel File Evidence
Each employee's Layer B file (F-QM-HR-004-A-V0) shall contain the following documentation:
| Record Category | Required Verification Documents |
|---|---|
| Identification & Appointment | Personal data sheet, appointment paper/contract copy, verified job assignment, and signed Position Description acknowledgment |
| Professional Credentials | Authenticated college diploma, official transcript of records, valid PRC registration card, and Board Certificate |
| Training & Competency | Completed Orientation Checklist (F-QM-HR-001-A-V0), Competency Evaluations (F-QM-HR-002-A-V0), and testing authorizations |
| Continuing Education | Annual CPD unit summary, certificates of seminar/workshop attendance, and BLS certification |
| Occupational Health | Baseline medical history, annual medical examination certificate, chest X-ray result, and immunization records |
5.3 File Custody, Maintenance, and Annual Currency Audit
- Physical Security: All active Layer B personnel folders shall be stored in a secured, locked cabinet within the Chief Medical Technologist's office to prevent unauthorized access.
- Annual Currency Review: The Chief MT / LQM shall audit every personnel file annually during the first quarter. Any lapsed PRC license, missing annual medical certificate, or expired BLS certification shall be flagged and resolved within thirty (30) calendar days.
- Data Privacy Compliance: In accordance with Republic Act No. 10173, access to personal information and health records is strictly restricted to the individual employee, Chief MT / LQM, Pathologist, and Hospital Director.
5.4 Staff Immunization and Health Surveillance
- Mandatory Immunizations: All laboratory workers exposed to clinical specimens and biohazardous waste shall receive Hepatitis B vaccination (3-dose series with post-vaccination titer confirmation) and annual Influenza immunization provided through hospital health services.
- Vaccine Declination: Staff members who refuse vaccination for documented medical or personal reasons shall sign an informed declination waiver acknowledging occupational risks. Declining staff may be subject to operational task reassignment during infectious disease outbreaks.
- Annual Medical Clearance: Laboratory personnel shall undergo an annual health assessment, including complete blood count, urinalysis, routine stool examination, and chest radiography.
5.5 Occupational Exposure Follow-Up and Work Restrictions
- Emergency First Aid: Immediate first aid (copious wound washing with soap and water, eye flushing for splashes) shall be performed immediately following any needlestick or mucosal exposure.
- Incident Routing: The exposure shall be documented on the Incident Report Form (Occurrence Management Policy) and referred within two (2) hours to the designated hospital emergency physician for clinical evaluation and post-exposure prophylaxis (PEP) initiation.
- Work Restrictions: Employees diagnosed with active communicable infections (e.g., active pulmonary tuberculosis, viral conjunctivitis, exudative skin lesions) shall be temporarily placed on medical work restriction. Reassignment to non-testing or administrative duties may occur until written return-to-work clearance is issued by the examining physician.
