1.0 Purpose
To establish an operational policy governing personnel recruitment, qualification verification, staffing complement planning, duty scheduling, workload review, and separation procedures for the Tarangnan Community Hospital (TCH) Clinical Laboratory. This policy ensures that the laboratory maintains an adequate complement of qualified, licensed, and competent personnel to meet diagnostic service demands, and that all transitions and separations are conducted with systematic handover and administrative clearance.
2.0 Scope
This policy applies to all clinical laboratory staff categories — Pathologist, Chief Medical Technologist / Laboratory Quality Manager, Medical Technologists, and Laboratory Aides — across full-time, contractual, Job Order (JO), reliever, and trainee arrangements. It defines the laboratory's interfaces with the municipal Human Resource Management Office (HRMO) for recruitment, duty roster posting, absence contingency, workload monitoring, voluntary resignation, disciplinary actions, and exit clearance.
3.0 Definitions
- Position Description — the controlled document defining the duties, responsibilities, reporting lines, qualifications, and authorization limits of a specific laboratory role.
- Job Order (JO) — government contractual engagement under CSC-COA-DBM Joint Circular No. 1 utilized for frontline operational staff.
- Staffing Complement — the approved baseline number and distribution of personnel required to operate laboratory sections safely and efficiently across all active shifts.
- Workload Capacity — the volume of diagnostic examinations and ancillary duties that can be accurately performed within standard turn-around times by available staff.
- Exit Clearance — the verified handover of pending testing duties, return of laboratory property and keys, revocation of LIS and facility access, and completion of documentation upon separation.
4.0 Responsibility
4.1 Hospital Director
- Approves the laboratory staffing plan, job descriptions, contract renewals, and formal appointments endorsed by hospital administration and the HRMO.
- Holds final authority over disciplinary actions (suspension, demotion, termination) and formal acceptance of resignations.
4.2 Pathologist
- Advises hospital leadership on clinical staffing requirements, technical qualifications, and workload adequacy to ensure examination quality and patient safety.
- Participates in candidate technical screening for Medical Technologist vacancies and oversees technical continuity during staff turnover.
4.3 Chief Medical Technologist / Laboratory Quality Manager
- Formulates position descriptions, reviews candidate credentials against DOH/PRC standards, and submits workload-based manpower requests to hospital management.
- Prepares and posts monthly duty schedules, manages shift coverage and leaves, conducts quarterly workload reviews, and oversees staff separations and clearance.
- Immediately removes separated personnel from the Authorization Matrix and revokes LIS and physical access permissions.
4.4 Medical Technologists
- Comply with posted duty schedules, maintain active PRC licensure and CPD compliance, and perform assigned technical examinations in accordance with standard operating procedures.
- Provide advance written notice for planned leaves or resignations, ensure thorough handover of ongoing testing, and complete the exit clearance checklist.
4.5 Laboratory Aides
- Fulfill assigned non-technical support schedules (cleaning, specimen transport, inventory handling, and temperature checks) under Medical Technologist direction.
- Provide timely notification of absences and complete exit clearance obligations upon conclusion of engagement; do not perform or interpret clinical examinations.
5.0 Policy / Procedure
5.1 Recruitment and Hiring Principles
- Staffing Input: The laboratory shall coordinate with the hospital HRMO to recruit personnel based on verified service needs, workload volume, and DOH licensing requirements.
- Qualification Verification: Prior to appointment, candidates shall submit authenticated proof of qualifications: - For Medical Technologists: Bachelor of Science in Medical Technology / Medical Laboratory Science, valid PRC professional registration card, and updated CPD compliance; - For Laboratory Aides: High school diploma or vocational health certificate, and relevant orientation in hospital utility or health support; - For Head of Laboratory: Clinical Pathologist certified by the Philippine Board of Pathology.
- Assessment of Applicants: A technical evaluation shall be conducted by the Chief Medical Technologist / Laboratory Quality Manager and Pathologist to assess procedural knowledge and operational suitability prior to HRMO recommendation.
5.2 Position Descriptions and Qualifications
- Written Job Descriptions: Controlled position descriptions shall be maintained for all laboratory roles (F-QM-HR-003-A-V0), specifying: - Official job title and organizational reporting relationship; - Specific clinical and technical duties; - Required educational background, PRC licensure, and experience; - Authority limits and mandatory training prerequisites.
- Staff Acknowledgment: Every employee shall read, understand, and sign an individual acknowledgment of their Position Description upon appointment and following any formal revision. Signed acknowledgments are maintained in the employee 201 file.
5.3 Promotion, Reassignment, and Staff Development
- Promotion Criteria: Recommendations for advancement or contract level upgrading shall be based on objective merit, including annual performance evaluation results (Competency Assessment Policy), demonstrated competency, punctuality, and commitment to the QMS.
- Reassignment: Internal reassignment between laboratory benches (Hematology, Clinical Chemistry, Microscopy, Serology) shall be accompanied by competency verification and an updated Authorization Matrix entry before independent shift testing.
5.4 Staffing Strategy and Complement Plan
- Approved Complement: The laboratory shall maintain an adequate staffing complement consistent with primary-category DOH standards (1 Pathologist, 1 Chief Medical Technologist / Laboratory Quality Manager, 4 Medical Technologists, and 2 Laboratory Aides).
- Workload-Based Sizing: Under DOH A.O. 2021-0037 Annex B1 Item 10, an increase in examination workload or test menu complexity requires a corresponding increase in laboratory personnel. Obsolete ratio formulas (e.g., fixed test-per-technologist quotas from A.O. 2007-0027) are replaced by empirical workload analysis.
- Staffing Review: The Chief MT / LQM shall evaluate staffing adequacy annually against specimen volumes, STAT turnaround times, and incident reports to formulate formal manpower budget requests.
5.5 Duty Scheduling, Attendance, and Workload Monitoring
- Posted Duty Roster: A monthly duty schedule (F-QM-HR-003-B-V0) shall be prepared by the Chief MT and posted at least one (1) week prior to the start of each coverage month. The schedule defines assigned shift hours, bench assignments, and designated on-call or reliever personnel.
- Absence Contingency: - Planned Leave: Staff must file leave requests at least two (2) weeks in advance to allow scheduled coverage adjustments; - Emergency / Sick Leave: Staff must notify the Chief MT at least two (2) hours before shift commencement. The designated reliever or off-duty technologist is activated according to the contingency roster; - Workload Surges: In the event of community outbreaks, mass casualty events, or sudden specimen spikes, off-duty personnel may be recalled on official emergency duty.
- Workload Analysis: Monthly test volume summaries and turnaround-time exceptions shall be reviewed to detect workload imbalances and initiate corrective operational shifts.
5.6 Voluntary Resignation and Separation
- Resignation Notice: Staff seeking voluntary resignation shall submit a formal written notice to the Chief Medical Technologist / Laboratory Quality Manager and Hospital Director at least thirty (30) calendar days prior to the effective date, unless an alternate notice period is stipulated in their Job Order contract.
- Duty Handover: During the notice period, the resigning staff member shall: - Complete and endorse all ongoing testing, pending QC reviews, and equipment maintenance logs; - Hand over section responsibilities, reagent inventory tallies, and assigned QMS tasks to designated incoming personnel; - Participate in an operational exit briefing with the Chief MT / LQM.
5.7 Disciplinary Action, Suspension, Demotion, and Termination
- Due Process: Any disciplinary measure resulting from performance deficiencies, gross negligence, deliberate protocol deviation, or misconduct shall strictly respect administrative due process: - Documented verbal counseling and fact-finding; - Formal written notice of charges with an opportunity to respond; - Written administrative resolution issued by hospital administration.
- Disciplinary Actions: Depending on the severity of the offense, progressive actions include written reprimand, official suspension, task demotion/authorization curtailment, or contract termination in accordance with CSC-COA-DBM Joint Circular No. 1 and Civil Service 2017 RACCS guidelines.
- Critical Safety Violations: Permitting unauthorized or uncertified individuals to perform diagnostic testing or falsifying patient records triggers immediate suspension and contract termination recommendation.
5.8 Laboratory Exit Clearance
- Prior to final release of wages, contract clearance, or issuance of certificates of employment, the departing employee shall complete the laboratory exit clearance checklist (F-QM-HR-003-A-V0, Part 3): - Verification of completed test handover and zero pending patient reports; - Surrender of physical keys, hospital identification badges, and personal protective equipment; - Deactivation of user access in the TCH LIS, shared laboratory email accounts, and electronic files; - Deletion of authorization from the active Personnel Authorization Matrix.
