1.0 Purpose
To establish an objective and unified system for assessing technical competency, granting task authorizations, and conducting periodic performance evaluations for all personnel in the Tarangnan Community Hospital (TCH) Clinical Laboratory. This policy ensures that staff demonstrate verified skills before performing diagnostic examinations independently, maintain high operational standards, and receive structured annual performance feedback and development support.
2.0 Scope
This policy applies to all Medical Technologists and Laboratory Aides performing routine, STAT, or support functions in the TCH Clinical Laboratory, including full-time, contractual, and Job Order (JO) staff. It defines initial and periodic competency verification, formal testing authorization, and annual performance appraisal integrated with hospital human resource management processes.
3.0 Definitions
- Competency Assessment — the systematic, objective evaluation of an employee's knowledge, technical skills, and practical adherence to standard operating procedures to confirm ability to perform assigned duties.
- Testing Authorization — the documented, formal privilege granted to an individual to perform specific diagnostic tests, operate analyzers, review quality control, and report laboratory results.
- Performance Evaluation — the periodic appraisal of an employee's overall job execution, reliability, quality commitment, teamwork, and adherence to hospital rules over a defined rating period.
- Development Plan — a structured, documented action plan identifying specific training, mentoring, or continuing education activities to enhance staff capability or address performance opportunities.
- Corrective Action Plan — a mandatory remediation program initiated when an employee fails to achieve the passing threshold on a competency assessment or demonstrates repeated procedural errors.
4.0 Responsibility
4.1 Hospital Director
- Approves the annual laboratory competency and performance evaluation schedule and provides necessary institutional resources.
- Reviews and endorses formal personnel actions resulting from performance evaluations (e.g., contract renewals, merit recognition, or administrative referrals).
4.2 Pathologist
- Assesses the technical competency of the Chief Medical Technologist / Laboratory Quality Manager and conducts peer review on clinical interpretation.
- Resolves technical disputes regarding competency evaluations, authorizes advanced technical procedures, and participates in annual performance appraisals.
4.3 Chief Medical Technologist / Laboratory Quality Manager
- Administers the laboratory competency assessment program, conducts direct observations, evaluates assessment records, and maintains the Authorization Matrix.
- Conducts annual performance evaluations for Medical Technologists and Laboratory Aides, formulates development plans, and oversees corrective retraining.
- Suspends testing authorization for staff members who fail competency reassessment until successful remediation is documented.
4.4 Medical Technologists
- Undergo scheduled competency assessments and participate constructively in annual performance evaluations.
- Perform only authorized laboratory procedures, adhere strictly to approved SOPs, and complete assigned development or remedial activities.
4.5 Laboratory Aides
- Undergo competency assessments specific to assigned non-technical support functions (cleaning, equipment surface decontamination, biohazard waste transport, temperature logging).
- Participate in annual performance appraisals and carry out assigned development tasks; do not perform, verify, or release clinical test results.
5.0 Policy / Procedure
5.1 Competency Assessment Frequency and Triggers
- Initial Assessment: Every newly appointed or Job Order Medical Technologist shall undergo formal competency assessment and receive written authorization before being permitted to perform clinical testing or release results independently (see Figure 1).
Figure 1 — Competency assessment, authorization, and retraining decision flow
- Periodic Reassessment: Competency assessments shall be conducted annually for all authorized technical procedures and support workflows.
- Specific Triggers: Immediate competency reassessment is mandatory under the following conditions: - Introduction of new test methods, analytical instruments (e.g., replacement hematology or chemistry analyzers), or significant SOP revisions; - Resumption of duties following an absence or gap in service exceeding three (3) months; - Identification of repeated analytical errors, quality control failures, or PT/EQAS nonconformities (Occurrence Management Policy); - Reassignment of staff to another laboratory section or bench.
5.2 Six Core Competency Evaluation Methods
In accordance with DOH and ISO 15189 standards, competency evaluations for testing personnel shall incorporate the following six methods across pre-analytical, analytical, and post-analytical phases:
| Method | Verification Activity | Evaluated Scope |
|---|---|---|
| 1. Direct Observation | Real-time observation of routine testing workflows | Specimen accessioning, pipetting, analyzer operation, slide preparation |
| 2. Result Monitoring | Systematic audit of recorded and reported patient values | Transcription accuracy, critical value telephoning, TAT compliance |
| 3. Quality Record Review | Audit of routine bench records | Daily calibration logs, internal QC charts, preventive maintenance sheets |
| 4. Procedural Knowledge | Assessment of SOP understanding | Oral questioning or written review of test principles and troubleshooting |
| 5. Practical Testing | Blind testing of internal controls or retesting | Blind split samples, duplicate analysis of known patient specimens, PT samples |
| 6. Problem Solving | Review of response to simulated or actual occurrences | Resolution of out-of-control QC runs, instrument error codes, sample rejections |
Color vision discrimination testing shall be documented for personnel performing visual macroscopic, microscopic, or colorimetric assessments (urinalysis dipsticks, peripheral blood smear evaluations).
5.3 Passing Standard, Remediation, and Corrective Action
- Passing Standard: The passing threshold for all written and practical competency evaluations is 75%.
- Failure and Authorization Suspension: If an employee scores below 75% or demonstrates an unsafe practice, testing authorization for that specific test or instrument shall be suspended immediately.
- Root Cause Check: Before assigning individual blame, the Chief Medical Technologist / Laboratory Quality Manager shall verify that SOPs, reagents, equipment, and training materials are clear, valid, and fully functional.
- Corrective Retraining: The preceptor or Chief MT shall administer targeted retraining using controlled SOPs and supervised practice.
- Reassessment: A formal reassessment shall be administered within ten (10) working days. Independent testing authorization shall only be reinstated upon achieving a passing score.
5.4 Personnel Authorization and Scope of Practice
- Formal Authorization: The laboratory shall maintain an up-to-date Personnel Authorization Matrix (F-QM-HR-002-B-V0).
- Granting Criteria: Testing authorization shall be granted by the Pathologist and Chief Medical Technologist / Laboratory Quality Manager only upon verified completion of: - Departmental orientation and LIS training (F-QM-HR-001-A-V0); - Current PRC professional license registration; - Satisfactory competency evaluation (F-QM-HR-002-A-V0).
- Immediate Revocation Warning: Under DOH A.O. 2021-0037 Annex C, allowing unauthorized, uncertified, or unregistered personnel to perform technical testing or release laboratory findings constitutes a critical regulatory violation carrying immediate LTO revocation. Laboratory Aides shall never be authorized to perform diagnostic examinations.
5.5 Annual Performance Evaluation and Review Cycle
- Annual Cycle: All laboratory personnel shall undergo formal performance evaluation at least once per evaluation year, aligned with the hospital's institutional appraisal schedule (IPCR).
- Evaluation Categories: Performance is rated across the following core areas: - Core Duties: Fulfillment of Position Description obligations and technical responsibilities; - Quality & Safety: Adherence to QMS policies, occurrence reporting, biosafety, and infection control; - Dependability & Efficiency: Punctuality, duty attendance, turn-around-time compliance, and shift handovers; - Interpersonal & Ethics: Professional conduct, teamwork, patient dignity, and confidentiality; - Continual Improvement: Active participation in audits, continuing education, and CQI projects.
- Appraisal Conference: The Chief Medical Technologist / Laboratory Quality Manager shall conduct a face-to-face feedback conference with the employee. The employee signs the evaluation to acknowledge receipt and may attach explanatory comments.
5.6 Performance Development Plans and Corrective Measures
- Development Planning: When an evaluation identifies skill gaps or career growth opportunities, a personalized Development Plan shall be formulated with specific objectives, assigned mentors, target completion dates, and progress review intervals.
- Substandard Performance: When an overall rating falls below satisfactory: - A written Performance Improvement Plan (PIP) shall be implemented for a duration of thirty (30) to sixty (60) days; - Technical deficiencies shall be cross-referenced to competency reassessment (Section 5.3); - Chronic uncorrected deficiencies or behavioral infractions shall be referred to hospital administration and HRMO per Internal Laboratory Rules and Regulations Policy.
5.7 Small-Laboratory Assessor Independence and Chief MT Evaluation
- To prevent conflict of interest in TCH's flat organizational structure, technical assessments of the Chief Medical Technologist / Laboratory Quality Manager shall be conducted directly by the Pathologist.
- The annual performance evaluation of the Chief MT / LQM shall be conducted jointly by the Hospital Director and the Pathologist.
