1.0 Purpose
The purpose of this policy is to establish a rigorous, transparent system for evaluating, selecting, approving, purchasing, receiving, and monitoring external providers of laboratory reagents, equipment, consumables, calibration, and support services at Tarangnan Community Hospital Clinical Laboratory. This ensures compliance with Philippine procurement laws and guarantees that all externally sourced products consistently meet diagnostic quality specifications.
2.0 Scope
This policy applies to all external providers of goods and services affecting laboratory operations (reagents, test kits, controls, calibrators, analytical equipment, maintenance contracts, metrology calibrations, EQA subscriptions, and referral laboratory testing), governing all personnel involved in purchasing, supply receiving, and vendor monitoring.
3.0 Definitions
- External Provider — any external commercial supplier, vendor, contractor, reference laboratory, or service organization delivering products or services to the laboratory.
- Approved External Provider List — an official register of evaluated and certified vendors authorized to supply products or technical services to the facility.
- Critical Supplies and Services — materials, instrumentation, or outsourced services whose failure directly degrades the quality or accuracy of diagnostic test results.
- Acceptance Inspection — technical verification of arriving deliveries against purchase orders, packaging integrity, temperature criteria, and regulatory documentation prior to inventory acceptance.
- Certificate of Product Registration (CPR) — official marketing authorization issued by the Philippine FDA confirming medical device and IVD reagent registration.
4.0 Responsibility
4.1 Hospital Director
- Approves institutional purchase orders, service contracts, and vendor agreements in compliance with Republic Act No. 9184 and Hospital Bids and Awards Committee (BAC) rules.
- Exercises executive oversight per Organization and Governance.
4.2 Pathologist
- Conducts clinical and technical review of proposed equipment, reagent specifications, and referral testing arrangements.
- Approves critical provider qualification criteria and evaluates significant vendor quality deficiencies per Organization and Governance.
4.3 Chief Medical Technologist / Laboratory Quality Manager
- Formulates technical specifications for annual procurement requisitions and maintains the Approved External Provider List.
- Evaluates supplier performance annually, manages delivery nonconformance reports, and initiates vendor corrective actions.
- Coordinates purchasing requests with the Hospital Supply Officer and Central Store.
4.4 Hospital Supply Officer / Central Store
- Facilitates purchase requisition processing, vendor purchase orders, logistics, and delivery receipt in coordination with laboratory leadership.
- Maintains central procurement files and delivery receipts.
4.5 Medical Technologists
- Perform technical receipt inspections for all incoming diagnostic shipments, verifying lot numbers, expiration dates, cold-chain indicators, and physical integrity.
- Document receipt logs and report damaged or nonconforming shipments immediately.
4.6 Laboratory Aides
- Assist in unpacking non-technical supplies and moving accepted packages to designated storage areas under Medical Technologist direction.
- Shall never perform technical delivery acceptance, sign receiving inspection logs, or evaluate vendor performance.
5.0 Policy / Procedure
5.1 Policy Statement
The laboratory shall procure products and services only from vetted, approved external providers meeting Philippine statutory requirements, FDA IVD device registration standards, and ISO 15189:2022 principles. All incoming critical materials shall undergo mandatory receipt inspection, and vendor performance shall be systematically monitored to ensure sustained quality and supply chain resilience.
5.2 Philippine Statutory Procurement Alignment
Public procurement of laboratory reagents, equipment, and services shall comply with Republic Act No. 9184 (Government Procurement Reform Act) and hospital procurement guidelines. Technical specifications developed by the Chief Medical Technologist shall be objective, non-restrictive, and focused entirely on analytical quality and clinical reliability.
5.3 Selection and Approval of External Providers
5.3.1 Selection Criteria
External providers shall be evaluated based on:
- Valid Philippine FDA License to Operate (LTO) as medical device / IVD distributor;
- Current FDA Certificate of Product Registration (CPR) for all quoted reagents and test systems;
- Product analytical reliability, precision, and clinical utility;
- Vendor technical support capability, delivery speed, and emergency breakdown response times;
- Pricing reasonableness and warranty coverage.
5.3.2 Provider Approval and Approved External Provider List
- Suppliers meeting all evaluation criteria shall be documented on the Approved External Provider List.
- Orders for critical materials and services shall be placed exclusively with vendors active on the approved list.
- The list shall be reviewed and updated annually by the Chief Medical Technologist.
5.4 Requisition and Annual Procurement Planning
- The Chief Medical Technologist shall establish an Annual Procurement Plan (APP) projecting reagent, consumable, and maintenance requirements based on historical test volumes and anticipated clinical demand.
- Routine purchase requests shall be initiated when inventory reaches established buffer reorder points to avoid stockouts.
- Paper purchase requests shall be routed through the Hospital Supply Officer for BAC processing.
5.5 Technical Receipt Inspection and Verification
5.5.1 Receiving Workflow
Upon physical delivery of reagents or consumables to the laboratory, the duty Medical Technologist shall immediately conduct an acceptance inspection:
- Verify shipping documents against the official Purchase Order (PO) and delivery invoice;
- Inspect shipping cartons for external crushing, leakage, puncturing, or tampering;
- Check cold-chain integrity and record transit temperature (ice pack condition or temperature data logger);
- Verify that every kit container bears an intact seal, legible lot number, manufacturer name, and unexpired shelf life (at least 6 months remaining shelf life upon receipt unless explicitly authorized);
- Verify that Philippine FDA CPR numbers are printed on outer packaging or accompanying documentation.
5.5.2 Receipt Logging and Documentation
- Every accepted delivery shall be recorded in the Receiving Verification section of the master registry.
- Containers shall be marked with the date received and the initials of the receiving Medical Technologist before being shelved in accordance with Reagent Management Policy (see Figure 1).

5.6 Nonconforming Deliveries and Discrepancies
- Deliveries failing acceptance criteria (compromised cold-chain, damaged vials, short-dated reagents, missing CPR) shall be rejected immediately.
- The Medical Technologist shall quarantine the shipment in a designated return holding area, clearly labeled DO NOT USE — REJECTED.
- The delivery discrepancy shall be documented on an occurrence report per Occurrence Management Policy, and the Hospital Supply Officer and vendor shall be formally notified for immediate credit or replacement.
5.7 Ongoing Monitoring and Periodic Re-Evaluation of External Providers
5.7.1 Performance Monitoring
Vendor performance shall be tracked continuously across four operational parameters:
- On-time delivery compliance;
- Product quality and frequency of damaged or nonconforming shipments;
- Responsiveness of technical and service personnel;
- Billing accuracy and documentation completeness.
5.7.2 Outcomes of Re-Evaluation
- Annual vendor evaluations shall be presented during Management Review meetings.
- Providers with satisfactory ratings shall be renewed on the Approved External Provider List.
- Providers with repeated nonconformances, delivery defaults, or unresolved service complaints shall be placed on conditional probation or permanently disqualified from the approved list by the Hospital Director upon Pathologist and Chief Medical Technologist recommendation.
