1.0 Purpose
The purpose of this policy is to establish an equipment management and environmental monitoring system for Tarangnan Community Hospital Clinical Laboratory. This governs equipment selection, installation qualification, inventory control, routine operation, preventive maintenance, calibration, downtime handling, decommissioning, and continuous temperature monitoring to guarantee uninterrupted, accurate diagnostic testing.
2.0 Scope
This policy applies to all analytical instrumentation, automated analyzers, centrifuges, microscopes, ancillary measuring devices, and temperature-dependent storage units (refrigerators, freezers, incubators) operated within the Clinical Laboratory, governing all Medical Technologists and Laboratory Aides.
3.0 Definitions
- Critical Equipment — analytical instrumentation and measuring devices directly affecting the accuracy, precision, or validity of laboratory examination results.
- Periodic Preventive Maintenance (PPM) — scheduled, systematic maintenance activities performed on equipment prior to operational failure to ensure sustained performance and prevent deterioration.
- Operational Verification — scheduled routine confirmation that an instrument continues to satisfy specified performance tolerances.
- Calibration — operation establishing a relationship between quantity values indicated by a measuring system and corresponding values provided by measurement standards.
- Temperature-Dependent Equipment — storage and analytical units whose functionality or stored contents require strict temperature control (refrigerators, freezers, water baths).

4.6 Laboratory Aides
- Record daily temperature readings on designated storage units and clean analyzer exterior surfaces under Medical Technologist direction.
- File service dossiers and equipment maintenance records under technologist supervision.
- Shall never perform technical instrument operation, calibration, internal preventive maintenance, or repair procedures.
5.0 Policy / Procedure
5.1 Policy Statement
The laboratory shall manage all diagnostic equipment and temperature-dependent units through a structured life-cycle program comprising documented selection, qualification, maintenance, calibration, environmental monitoring, and decommissioning. Equipment shall be maintained in optimal operational condition, and temperature-dependent storage devices shall be monitored continuously to preserve specimen and reagent integrity.
5.2 Equipment Selection, Procurement, and Master Inventory
- Equipment selection shall be coordinated by the Chief Medical Technologist and Pathologist, evaluating clinical utility, reagent availability, power requirements, and vendor service capability.
- A master equipment inventory shall be maintained in the laboratory dossier and the non-interfaced TCH LIS, capturing: - Unique equipment identification number and asset tag; - Equipment description, manufacturer, model, and serial number; - Physical location in the laboratory; - Date of delivery and date placed into clinical service; - Servicing vendor and contact details (SAN PHARMA COMPANY); - Current operational status (In Service, Out of Service, Under Repair, Standby).
5.3 Installation Qualification and Commissioning
- Newly acquired, relocated, or extensively repaired equipment shall undergo formal Installation Qualification (IQ) and Operational Qualification (OQ) prior to clinical testing.
- The authorized distributor technician and Chief Medical Technologist shall verify electrical grounding, environmental conditions, software configuration, and mechanical tolerances.
- Method performance verification (precision, accuracy, reportable range) shall be completed in accordance with Method Validation Policy before patient testing begins.
5.4 Equipment Identification and Usability Status
- Each piece of equipment shall be prominently labeled with its unique inventory code.
- Equipment operational status shall be communicated via standardized color-coded status tags: - Green Tag (IN SERVICE): Fully operational, calibrated, and authorized for clinical testing. - Yellow Tag (AWAITING CALIBRATION / CONDITIONAL): Restricted use or calibration due; requires verification before patient runs. - Red Tag (OUT OF SERVICE / DEFECTIVE): Defective, malfunctioning, or uncalibrated; strictly quarantined from diagnostic use.
5.5 Operation and Authorized Personnel
- Only trained and competency-assessed Medical Technologists shall operate analytical instrumentation.
- Current, manufacturer-approved operator manuals, quick-reference guides, and technical SOPs shall be maintained readily accessible at each workstation.
5.6 Preventive and Routine Maintenance
- Maintenance activities are categorized into two tiers: - Operator Maintenance: Routine daily, weekly, and monthly cleaning, optical cleaning, probe flushing, and filter checks performed by Medical Technologists and documented on section maintenance checklists. - Vendor Preventive Maintenance (PPM): Scheduled semi-annual or annual servicing, electrical safety testing, sub-assembly replacements, and alignment performed by the authorized distributor (SAN PHARMA COMPANY).
- Service reports detailing PPM findings and parts replacement shall be filed in the equipment dossier.
5.7 Calibration, Calibration Verification, and Metrological Traceability
- Measuring systems and analytical analyzers shall be calibrated at defined intervals using certified calibrators traceable to national or international reference materials (NIST or WHO standards).
- Recalibration shall be performed when: - Routine quality control indicates an unresolved systematic shift or drift; - Major component replacement or optical realignment occurs; - Recommended by the manufacturer's package insert or service schedule.
- Calibration verification shall confirm accuracy across the reportable range at least semi-annually.
- If calibration cannot be achieved or falls out of acceptable limits, testing shall be discontinued immediately, the unit tagged Red (OUT OF SERVICE), and the Chief Medical Technologist notified.
5.8 Equipment Temperature Monitoring and Acceptable Ranges
- All temperature-dependent storage units and ambient workspace areas shall be monitored and logged twice daily (every 12 hours) on the Master Temperature Log.
- Standardized operational temperature thresholds:
| Equipment / Area | Acceptable Temperature Range | Monitoring Device |
|---|---|---|
| Reagent & Specimen Refrigerators | 2 °C to 8 °C | Calibrated digital / spirit thermometer |
| Specimen & Control Freezers | −15 °C to −25 °C | Calibrated digital / probe thermometer |
| Incubators and Water Baths | 36.5 °C to 37.5 °C | Calibrated immersion thermometer |
| Ambient Laboratory Workspace | 20 °C to 25 °C (Humidity 30%–70%) | Calibrated thermo-hygrometer |
- Temperature Excursion Troubleshooting: - If an out-of-range temperature is observed, silence any active alarm and inspect immediate mechanical causes (door ajar, power disconnection, heavy frost buildup). - If correctable, monitor temperature every 15 minutes for 1 hour. - If temperature fails to stabilize within 1 hour, immediately transfer stored reagents and specimens to an alternate qualified refrigerator or insulated thermal container with ice packs. - Tag the unit OUT OF SERVICE and contact SAN PHARMA COMPANY for technical repair.
5.9 Malfunction, Out-of-Service Protocols, and Decommissioning
- Any instrument exhibiting defective performance, optical errors, repeated QC failures, or physical damage shall be removed from service immediately and tagged with a Red OUT OF SERVICE tag.
- Patient specimens tested between the last satisfactory check and failure shall be reviewed for validity; compromised results shall be amended per Occurrence Management Policy.
- Before returning a repaired unit to service, Medical Technologists shall perform recalibration, precision checks, and routine QC testing to confirm satisfactory operational status (see Figure 1).
Figure 1 — Equipment malfunction tag-out, repair, and return-to-service verification flow
- Decommissioned or obsolete equipment shall be bio-decontaminated, removed from clinical areas, and marked retired in the master inventory.
5.10 Downtime Communication and Incident Escalation
- In the event of primary equipment breakdown, the Chief Medical Technologist shall notify clinical hospital units of estimated testing delays.
- Urgent diagnostic specimens shall be triaged, stabilized, and referred to Samar Provincial Hospital in accordance with Referral Laboratory Policy.
- All downtime events and repair chronologies shall be documented in the equipment service log.
