1.0 Purpose
The purpose of this policy is to establish an effective system for managing reagents, laboratory water, supplies, and consumables at Tarangnan Community Hospital Clinical Laboratory. This ensures that only verified, properly stored, and unexpired materials supporting reliable diagnostic testing are placed into analytical service.
2.0 Scope
This policy applies to all reagents, kits, controls, calibrators, consumables, and laboratory-grade water used across all sections of the Clinical Laboratory, and governs the inventory, verification, storage, and handling duties of all laboratory personnel.
3.0 Definitions
- Critical Reagents and Consumables — materials and supplies essential for testing whose quality directly impacts the reliability of diagnostic examination results (e.g., test kits, staining solutions, controls, calibrators, and analyzer consumables).
- Clinical Laboratory Reagent Water (CLRW) — purified water meeting defined chemical and microbiological purity specifications suitable for clinical laboratory analytical methods.
- First-Expiry, First-Out (FEFO) — an inventory management practice ensuring that materials with the shortest remaining shelf life are utilized before newer stock.
- Lot-to-Lot Verification — confirmation that a newly received lot number of reagents performs comparably to the active lot before being used for patient testing.
- Certificate of Product Registration (CPR) — official marketing authorization issued by the Philippine Food and Drug Administration (FDA) certifying IVD reagent compliance.
4.0 Responsibility
4.1 Hospital Director
- Approves annual laboratory supply budgets and procurement contracts through the Hospital Bids and Awards Committee.
- Exercises executive oversight per Organization and Governance.
4.2 Pathologist
- Provides clinical and technical oversight for reagent acceptance criteria, lot-to-lot verification thresholds, and disposal approvals.
- Exercises clinical governance per Organization and Governance.
4.3 Chief Medical Technologist / Laboratory Quality Manager
- Maintains the master inventory of critical reagents, consumables, and commercial distilled water supplies.
- Validates supplier compliance, reviews lot-to-lot verification data, and enforces quarantine of defective or expired supplies.
- Monitors stock levels and coordinates timely replenishment with the Hospital Supply Officer.
4.4 Engineering and Maintenance Personnel (External / Facilities-Support)
- Facilitate plumbing repairs and ensure municipal tap water delivery to the laboratory sink.
- Assist in coordinating bulk deliveries of commercially purchased 10L container distilled water.
4.5 Medical Technologists
- Inspect incoming reagents, verify packaging integrity, storage temperatures, and expiration dates upon receipt.
- Label reagents with receipt date, date opened, and revised in-use expiration date where applicable.
- Conduct and document lot-to-lot verification studies prior to clinical utilization.
4.6 Laboratory Aides
- Assist in unpacking, stocking, and shelving supplies in accordance with FEFO placement under Medical Technologist direction.
- Monitor and log storage room and refrigerator temperatures daily.
- Shall never perform reagent lot-to-lot validation, technical acceptance decisions, or water quality testing.
5.0 Policy / Procedure
5.1 Policy Statement
The laboratory shall maintain a controlled inventory system ensuring that all reagents, calibrators, controls, consumables, and water meet strict quality standards. All reagents shall be verified for integrity upon receipt, labeled with traceability dates, stored within validated environmental limits, and used strictly within established manufacturer stability limits. Expired reagents shall be quarantined and disposed of immediately.
5.2 Storage of Supplies and Reagents
- Reagents, controls, and test kits shall be stored strictly within manufacturer-stipulated temperature ranges: - Refrigerated materials: 2 °C to 8 °C; - Frozen materials: −15 °C to −25 °C (or ultralow where required); - Room-temperature materials: 20 °C to 25 °C.
- Storage temperatures shall be monitored and recorded twice daily (every 12 hours) in accordance with Equipment Temperature Monitoring Policy.
- Reagents shall be organized following 5S principles and the First-Expiry, First-Out (FEFO) inventory sequence.
- Reagents shall never be stored directly on floors; shelving units, ventilated cabinets, and dedicated cold-storage units shall be utilized.
- In accordance with DOH A.O. 2021-0037 Annex C, under no circumstances shall expired reagents be retained in analytical work areas or used for diagnostic testing. Expired materials shall be segregated immediately into a designated quarantine area.
5.3 Inventory Management and Stock Cards
- The laboratory shall maintain an active inventory record on physical stock cards and within the TCH LIS.
- Each inventory item shall record: - Brand name, catalog number, and manufacturer; - Lot number and manufacturer expiration date; - Unit of issue, date received, quantity received, and storage location; - Date opened and assigned in-use expiration date; - Minimum buffer stock level and reorder point.
- Monthly physical counts shall be conducted by the Chief Medical Technologist to reconcile inventory and prevent stockouts.
5.4 Reagent Labeling and Traceability
- All reagent containers, prepared working solutions, and aliquot vials shall bear legible labels containing: - Reagent identity, concentration, and lot number; - Date received, date opened, and initials of the opening technologist; - Storage requirements and in-use expiration date.
- Reagents from different kit lots shall not be interchanged or mixed unless explicitly authorized by the manufacturer's package insert.
5.5 Laboratory Reagent Water Program
- Analytical equipment and procedural protocols requiring distilled water at TCH utilize commercially purchased, sealed 10L containers of laboratory-grade distilled water (CLRW grade).
- Each newly delivered lot or batch of commercial distilled water shall be inspected for container seal integrity, clarity, and expiration date.
- Tap water from the laboratory sink shall be restricted to handwashing, primary cleaning, and non-analytical rinsing. Final rinsing of reusable labware shall utilize Type 3 distilled water.
5.6 Validation of New Lots and/or Shipments
- Prior to placing a new lot of reagents or test kits into routine clinical use, lot-to-lot verification shall be performed by Medical Technologists.
- Verification methods: - Quantitative tests: Analyze current controls and at least 3 patient specimens across low, normal, and high ranges in parallel on both old and new lots. Results shall fall within manufacturer acceptance limits. - Qualitative tests: Analyze known negative, weakly positive, and strong positive controls on both lots to verify concordant reactivity.
- Verification results shall be reviewed and authorized by the Chief Medical Technologist / Laboratory Quality Manager prior to releasing the lot for patient testing.
5.7 Expired Reagents, Deficiencies, and Disposal
- Any reagent failing lot-to-lot verification, exhibiting physical deterioration (turbidity, precipitation, discoloration), or reaching expiration shall be quarantined immediately.
- Disposal shall be logged on the waste manifest in accordance with healthcare waste management standards and Laboratory Safety Policy.
6.0 Records
The following controlled record is maintained under this policy:
| Form No. | Form Title | Parent Policy/SOP | Type | Status |
|---|---|---|---|---|
| Electronic record | Reagent, Water, and Consumables Inventory, Lot Verification & Tracking Log | LAB-QM-ANA-002 | Register / Log | Electronic — registered under F-QM-002-B-V0 |
Records shall be retained in the department archive in accordance with Laboratory Retention Schedule Policy.
These records are maintained electronically in analyzer memory, the LIS, and dedicated electronic workbooks registered under F-QM-002-B-V0 (Decision D5); no separate paper form is maintained for them.
