1.0 Purpose
This policy establishes a formal, systematic process for executive review of the Clinical Laboratory Quality Management System (QMS) at Tarangnan Community Hospital (TCH). It ensures that Laboratory Management regularly evaluates the continuing suitability, adequacy, and effectiveness of laboratory operations, quality policies, and objectives, providing leadership decisions and resource allocations to improve clinical services and patient care.
2.0 Scope
This policy applies to all Management Review activities of the TCH Clinical Laboratory QMS, encompassing governance, pre-examination, examination, and post-examination operations, point-of-care testing (POCT), personnel competence, equipment and facilities, risk management, internal audits, CAPAs, and continual improvement programs.
3.0 Definitions
- Management Review — a formal, planned evaluation conducted by Laboratory Management to assess the performance, suitability, adequacy, and effectiveness of the QMS and establish strategic improvement actions.
- Review Input — objective data, operational reports, audit findings, performance metrics, and feedback presented for evaluation during Management Review.
- Review Output — the formal decisions, resource allocations, action assignments, and policy updates resulting from Management Review.
- Action Item — a specific task assigned during Management Review with a designated owner, required resources, target completion date, and verification mechanism.
- QMS Effectiveness — the extent to which planned laboratory activities are realized and planned quality objectives achieved.
4.0 Responsibility
4.1 Hospital Director
- Chairs and leads Management Review meetings as the executive authority of the hospital.
- Evaluates overall laboratory performance, authorizes required institutional resources (budget, staffing, facility maintenance), and aligns laboratory objectives with hospital-wide strategic goals.
4.2 Pathologist
- Co-leads Management Review, providing clinical and diagnostic evaluation of laboratory quality data, examination validity, and patient-safety impact.
- Reviews and authorizes technical decisions, clinical quality targets, and diagnostic testing improvements.
4.3 Chief Medical Technologist / Laboratory Quality Manager
- Coordinates the Management Review process: schedules meetings, compiles operational data, prepares the agenda, and documents minutes and action items in F-QM-013-A-V0.
- Monitors progress on assigned review actions, conducts follow-up verifications, and communicates decisions to laboratory staff.
4.4 Medical Technologists
- Compile section-specific data (IQC summaries, EQAS reports, equipment logs, test turnaround times, and incident reports) as inputs for Management Review.
- Implement assigned technical decisions and workflow modifications resulting from review outputs.
4.5 Laboratory Aides
- Provide environmental temperature logs, facility maintenance notes, and supply storage data within their assigned duties.
- Implement non-technical support directives resulting from review decisions.
- Laboratory Aides do not evaluate technical performance, participate in clinical review, or authorize QMS decisions.
5.0 Policy / Procedure
5.1 Review Principles
- Executive Leadership & Accountability: Management Review shall be led jointly by the Hospital Director, Pathologist, and Chief Medical Technologist / Laboratory Quality Manager.
- Full and Evidence-Based: The review shall evaluate objective data from all QSE domains across the entire laboratory workflow rather than assessing individual sections in isolation.
- Action-Oriented: Every Management Review shall yield documented, actionable decisions with assigned responsibilities, clear timelines, and dedicated resources tracked to verified completion (see Figure 1).

5.2 Frequency, Scheduling, and Attendees
- Planned Frequency: Management Review shall be conducted at planned intervals at least annually, or semi-annually when indicated by major operational changes.
- Special / Extraordinary Reviews: An extraordinary Management Review may be convened upon major triggers: commissioning of major analyzers, significant regulatory or DOH licensing updates, critical patient-safety incidents, or sentinel events.
- Participants: Mandatory attendees are the Hospital Director, Pathologist, and Chief Medical Technologist / Laboratory Quality Manager. The designated Laboratory Safety Officer participates when biosafety or facility matters are on the agenda. Key Medical Technologists may be invited to present section findings.
5.3 Planning and Agenda Preparation
- Advance Notice: The Chief Medical Technologist / Laboratory Quality Manager shall prepare the meeting schedule and distributed agenda at least two weeks in advance using F-QM-013-A-V0.
- Information Compilation: Section data, trend analyses, and CAPA summaries shall be compiled and verified prior to the meeting.
5.4 Review Inputs
Management Review shall evaluate objective information covering all mandatory QMS elements:
- Status of action items from prior Management Reviews;
- Changes in internal and external factors (regulations, hospital policies, test workload, and clinical user needs);
- Quality objectives fulfilment and quality indicator trends under Continual Improvement and Performance Improvement Program Policy;
- Internal audit findings under Internal Audit and external regulatory inspections (DOH, PhilHealth);
- Summary of occurrences, deviations, and complaints under Occurrence Management Policy;
- Status, effectiveness, and recurring trends of CAPAs under Corrective and Preventive Action Policy;
- Assessment of result validity: Internal Quality Control (IQC) and External Quality Assessment Scheme (EQAS) performance;
- Evaluation of equipment reliability (HORIBA Pentra XLR, chemistry analyzer), maintenance logs, and reagent/consumable inventory;
- Performance of referral testing services (Samar Provincial Hospital);
- Staffing levels, training, and personnel competency records;
- Evaluation of Point-of-Care Testing (POCT) quality;
- Review of the Laboratory Risk Register and biosafety controls under Risk Management.
5.5 Conduct of Review & Decision-Making
- Structured Evaluation: The meeting proceeds through each agenda item systematically, evaluating whether current processes remain effective and compliant.
- Collaborative Problem-Solving: Management discusses operational bottlenecks, resource shortages, and patient-safety risks, agreeing on root solutions rather than superficial adjustments.
5.6 Review Outputs
The outputs of Management Review shall be documented in F-QM-013-A-V0 and shall include decisions and resource commitments addressing:
- QMS Suitability and Effectiveness: Overall assessment of whether the management system achieves its quality policy and objectives;
- Service and Quality Improvements: Concrete projects to enhance test turnaround times, diagnostic accuracy, and specimen handling;
- Resource Allocations: Executive approval for budget, equipment purchases or service contracts, reagents, facility modifications, and staffing;
- Policy & SOP Revisions: Necessary updates to controlled policies and procedures per Document Control Policy;
- Risk and Safety Adjustments: Modifications to risk mitigation plans and safety equipment.
5.7 Review Actions, Communication, and Follow-up
- Action Tracking: Every output requiring implementation shall be entered into the Action Follow-Up Log section of F-QM-013-A-V0, defining the specific action, responsible person, target completion date, and verification method.
- Staff Communication: The Chief Medical Technologist / Laboratory Quality Manager shall summarize key conclusions, improvement priorities, and approved resources for all Medical Technologists and Laboratory Aides during the next monthly laboratory meeting.
- Follow-Up and Verification: The Chief Medical Technologist / Laboratory Quality Manager tracks open actions monthly. Incomplete or overdue items remain on the log until verified complete and presented at the subsequent Management Review.
