1.0 Purpose
This policy establishes a unified Corrective and Preventive Action (CAPA) system for the Tarangnan Community Hospital (TCH) Clinical Laboratory. It defines structured procedures to investigate detected nonconformities, determine underlying root causes, implement corrective actions to prevent recurrence, proactively identify emerging vulnerabilities to prevent occurrence, and evaluate the effectiveness and sustainability of all implemented actions.
2.0 Scope
This policy applies to all Medical Technologists and Laboratory Aides across all sections of the TCH Clinical Laboratory. It governs corrective action triggered by occurrences, deviations, internal audits, EQAS failures, or complaints, as well as preventive action triggered by risk assessments, unfavorable IQC/indicator trends, or near-misses across pre-examination, examination, and post-examination processes.
3.0 Definitions
- Corrective Action — action taken to eliminate the root cause of a detected nonconformity, occurrence, or deviation to prevent its recurrence.
- Root Cause — the fundamental, underlying system deficiency that allowed a nonconformity to occur, distinct from immediate symptoms or contributing factors.
- Correction — immediate containment or remediation taken to repair or contain a detected error or its consequences without addressing the underlying root cause.
- Potential Nonconformity — an identified vulnerability, unfavorable trend, or near-miss that could result in an actual failure if not proactively mitigated.
- Preventive Action — proactive action taken to eliminate the cause of a potential nonconformity or mitigate risk before an adverse event occurs.
- Effectiveness — objective evidence verifying that an implemented corrective action has eliminated the root cause without recurrence, or that a preventive action has successfully prevented failure.
4.0 Responsibility
4.1 Hospital Director
- Approves this policy and allocates institutional resources required to implement systemic corrective actions and preventive infrastructure upgrades.

- Periodically trends CAPA data and reports outcomes to the CQI Committee and Management Review.
4.4 Medical Technologists
- Participate actively in root-cause investigations and implement assigned technical corrective and preventive measures within agreed timeframes.
- Monitor analytical processes, identify unfavorable IQC or instrument trends, and report when an implemented CAPA fails to resolve the underlying vulnerability.
4.5 Laboratory Aides
- Implement assigned non-technical corrective and preventive measures (e.g., specimen reception protocols, environmental temperature monitoring, storage maintenance) within their job scope.
- Immediately report process barriers, facility issues, or recurring support problems to a Medical Technologist or the Chief Medical Technologist.
- Laboratory Aides do not perform, verify, authorize, interpret, or release technical examinations or results.
5.0 Policy / Procedure
5.1 Policy Statement & CAPA Framework
- Integrated Framework: The Clinical Laboratory shall maintain a systematic CAPA framework that operates both reactively (to eliminate root causes of detected nonconformities) and proactively (to prevent potential nonconformities from occurring).
- Proportionality: The depth of root-cause investigation and the scale of corrective or preventive action shall be proportionate to the risk to patient safety, clinical impact, and frequency of occurrence.
- Evidence-Based and Sustainable: Every CAPA shall be supported by verifiable objective evidence, tracked in F-QM-011-A-V0, and made part of routine operations through standard operating procedure revisions and staff training (see Figure 1).

5.2 Initiation and Screening
- Triggering Sources: - Corrective Action Triggers: Major occurrences or deviations reported under Occurrence Management Policy, recurring minor errors, nonconformities from internal audits (Internal Audit), external inspection findings, EQAS/PT failures, or repeated QC rejections. - Preventive Action Triggers: Risk assessment findings under Risk Management, adverse IQC/calibrator drift trends, quality indicator variances under Continual Improvement and Performance Improvement Program Policy, recurring near-misses, or equipment preventative maintenance warnings.
- Screening and Triage: The Chief Medical Technologist / Laboratory Quality Manager screens incoming triggers to verify whether immediate containment is sufficient or whether formal CAPA is required. Issues requiring broader quality enhancement rather than fault correction are routed to continual improvement.
5.3 Root-Cause Analysis
- Investigation Methodology: For every nonconformity requiring CAPA, the Chief Medical Technologist / Laboratory Quality Manager and assigned Medical Technologists shall conduct a structured root-cause analysis documented in F-QM-011-A-V0: - 5 Whys Analysis: Used for straightforward, single-factor occurrences. The team asks "Why?" iteratively until arriving at the root deficiency in policy, training, or process control within the laboratory's authority to correct. - Cause-and-Effect (Fishbone / Ishikawa) Analysis: Used for complex, systemic, or recurring nonconformities. Causes are evaluated systematically across six core domains: Personnel (competency, fatigue, supervision), Equipment (maintenance, calibration, breakdown), Materials (reagents, calibrators, tube lots), Methods (written SOP clarity, sequence), Measurement (detection limits, QC rules), and Environment (temperature, lighting, workload).
- Systemic Scope: The analysis shall determine whether the identified root cause exists or could potentially occur in other laboratory sections or examination benches. Newly discovered hazards shall be referred to Risk Management.
5.4 Corrective Action Planning and Approval
- Action Plan Content: The CAPA plan in F-QM-011-A-V0 shall specify: - Specific corrective actions addressing the verified root cause; - Designated action owner and target completion date; - Required financial, equipment, or training resources; - Defined success criteria and effectiveness evaluation methodology.
- Review and Approval: Corrective action plans involving analytical procedures, clinical interpretation, or patient safety require review and written approval by the Pathologist. Plans requiring hospital budget allocations or administrative policy adjustments require approval by the Hospital Director.
5.5 Implementation and Effectiveness Review
- Action Execution: The assigned action owner shall implement the approved measures within the target timeframe, compiling objective evidence (e.g., revised SOPs, staff re-training records, maintenance service receipts, or revised bench aids).
- Effectiveness Evaluation: After an appropriate monitoring period (typically 30 to 90 days), the Chief Medical Technologist / Laboratory Quality Manager shall evaluate objective performance data (subsequent IQC/EQAS runs, quality indicator data, audit re-checks, occurrence logs) to verify that: - The root cause has been effectively eliminated; - The nonconformity has not recurred; - No unintended new risks or process bottlenecks were created.
- Closure and Re-assessment: If effective, the Pathologist or Chief Medical Technologist / Laboratory Quality Manager closes the CAPA in F-QM-011-A-V0. If ineffective, the issue is re-opened for deeper root-cause investigation and revised action planning.
5.6 Preventive Action: Identification and Evaluation of Potential Problems
- Proactive Surveillance: The laboratory shall actively review operational and performance data to detect vulnerabilities before failures occur: - Reviewing IQC Levey-Jennings charts and calibration trends to detect analytical drift prior to run failure (DOH QMS Tool Item 4.1.6); - Tracking quality indicator variation and turnaround time trends (DOH QMS Tool Item 5.5.1); - Analyzing near-miss reports and staff suggestions; - Reviewing supplier reliability and ambient room/refrigerator temperature logs.
- Risk-Based Evaluation: Potential problems shall be evaluated based on their probability of occurrence and clinical severity. When a preventive measure is warranted, an action plan is documented in F-QM-011-A-V0.
5.7 Preventive Measure Selection, Memos, Institutionalization, and Trending
- Management Approval and Directives: Proposed preventive measures shall be submitted to the Pathologist and Hospital Director for review (DOH QMS Tool Item 5.5.2). When approved preventive measures modify workflows or laboratory rules, formal administrative memoranda shall be issued to direct staff implementation (DOH QMS Tool Item 5.5.3).
- Preventive Action Plan: The preventive plan shall define specific controls, responsible personnel, deadlines, and effectiveness indicators (DOH QMS Tool Item 5.5.4).
- Institutionalization and Staff Orientation: Upon verified effectiveness, preventive measures shall be made permanent through controlled SOP revisions per Document Control Policy and full staff orientation (DOH QMS Tool Item 5.5.5).
- Periodic Review and Trending: The Chief Medical Technologist / Laboratory Quality Manager shall conduct quarterly trend reviews of all open and closed CAPAs to identify systemic patterns and overdue items (DOH QMS Tool Item 5.2.3). Analysis summaries shall be submitted to the CQI Committee and presented during annual Management Review (Management Review).
