5.0 Policy / Procedure
5.1 Ethical Principles, Patient Rights, and Welfare
Patient well-being, safety, and rights shall be the primary consideration in all laboratory activities. All personnel shall uphold honesty, scientific integrity, and non-discrimination. In support of patients and clinical users, the laboratory shall:
- Treat all patients, clinical specimens, and human remains with due care, dignity, and respect, providing care free from discrimination.
- Provide patients and clinicians with clear information on available examinations, preparation requirements, fees (where applicable), and expected turnaround times, and allow patients and clinicians to provide clinical information that aids test selection and result interpretation (per Scope of Service and Laboratory User Information Policy).
- Obtain informed consent where required by procedure (per Specimen Collection Policy) and periodically review the test menu with the Pathologist to confirm clinical appropriateness.
- Make relevant laboratory information available to the patient or their authorized healthcare provider upon request in accordance with Confidentiality Policy.
- Disclose to patients, clinicians, and relevant authorities any incident that caused or could have caused patient harm, documenting mitigation actions under Occurrence Management Policy and Corrective and Preventive Action Policy.
- Ensure the ongoing integrity and availability of retained patient specimens and records in the event of laboratory closure, merger, or restructuring per Control of Records and Laboratory Retention Schedule Policy.
5.2 Impartiality and Conflict of Interest
- Safeguarding Impartiality: Laboratory management is committed to total impartiality. Commercial, financial, political, or personal pressures shall never be allowed to influence test selection, analytical execution, result interpretation, or procurement. No rebates, commissions, or referral inducements shall be solicited or accepted.
- Monitoring and Mitigation of Threats: The Chief Medical Technologist / Laboratory Quality Manager shall continually monitor laboratory activities and staff relationships (including relationships connected to governance, shared resources, contracts, supplies, or external referrals) for threats to impartiality. When a threat is identified, documented measures (such as task reassignment, dual review, or recusal) shall be implemented immediately to eliminate or minimize the threat.
- Conflict-of-Interest Disclosure: Personnel shall disclose any actual or potential conflict of interest in writing to the Chief Medical Technologist / Laboratory Quality Manager before participating in the affected activity. Disclosures and management decisions shall be retained in the employee's personnel dossier per Control of Records.

5.4 Working Hours, Breaks, Leave, and Staff Meetings
- Working Hours and Attendance: Medical Technologists and Laboratory Aides work 40 hours per week totaling 176 hours per month according to the posted duty schedule. Staff shall be punctual and sign the daily attendance record. When a staff member is absent, on-duty personnel shall coordinate coverage so laboratory operations remain uninterrupted.
- Lunch Break Coverage: The designated meal break is 12:00 noon to 1:00 PM. Because the Clinical Laboratory remains open continuously, lunch breaks shall be staggered whenever more than one staff member is on duty, and STAT specimens must be completed or handed over before a staff member goes on break.
- Sick and Vacation Leave: Sick leave (supported by any legitimate medical certificate) and vacation/annual leave (submitted with advance notice and approved by HR/Administration) follow the staffing and leave coverage rules in Recruitment Qualification and Appointment Policy and formal delegation rules in Organization and Governance.
- Monthly Staff Meetings: The Chief Medical Technologist / Laboratory Quality Manager or Pathologist shall hold a laboratory staff meeting at least monthly (or as needed) to review operational issues, quality indicators, and policy updates. Minutes and signed attendance records shall be maintained, and absent staff shall read and sign the circulated minutes upon return.
5.5 Core Bench Workflow Rules
- Specimen Integrity: Never analyze an unlabeled, mislabeled, or improperly collected specimen, or any specimen failing the two-patient-identifier rule. Log all rejected specimens immediately in F-QM-PRE-003-A-V0 and notify the requesting unit for recollection per Specimen Collection Policy.
- STAT Escalation and Serious Errors: Follow up delayed or problematic STAT tests immediately with the requesting physician. Immediately report any critical error (such as a wrong patient result, false critical value, or major analyzer malfunction) to the Chief Medical Technologist / Laboratory Quality Manager and Pathologist, and log it under Occurrence Management Policy.
- Equipment, Reagents, Safety, and Waste: Perform daily analyzer maintenance per manufacturer manuals and tag any defective machine "Out of Order". Follow Reagent Management Policy for FIFO stock rotation, Laboratory Safety Policy for biosafety, and Hand Hygiene Policy for hand hygiene and healthcare waste segregation.
- Training and SOP Access: Current SOPs and work instructions shall remain accessible at each workbench. All trainees shall complete orientation, supervised training, and competency assessment per Orientation Policy and Competency Assessment Policy.
5.6 Reporting Ethical Concerns, Non-Retaliation, and Disciplinary Referral
- Good-Faith Reporting and Non-Retaliation: All personnel are expected to report suspected unethical conduct, data manipulation, threats to impartiality, or quality occurrences promptly to the Chief Medical Technologist / Laboratory Quality Manager or Pathologist. Personnel who report concerns or errors in good faith are protected from retaliation; good-faith occurrence reporting is a quality-improvement duty and shall not trigger automatic punishment.
- Disciplinary Framework and Due Process: Confirmed misconduct, repeated SOP violations, or breaches of professional ethics shall be addressed under the progressive counseling, written warning, minimum due-process (written notice to explain, opportunity to be heard, and written decision for Job Order staff under CSC-COA-DBM Joint Circular No. 1; or CSC RACCS referral for any regular/plantilla appointee), and administrative escalation procedures defined in Recruitment Qualification and Appointment Policy. Quality-system nonconformities arising from misconduct shall also be investigated under Occurrence Management Policy and Corrective and Preventive Action Policy.
- Annual Review: Compliance with ethical standards, impartiality safeguards, and internal rules shall be reviewed annually during Management Review (Management Review).
