5.2 CQI Committee and Annual Study Schedule
- Designation and Composition: The Hospital Director shall designate in writing a Continual Quality Improvement (CQI) Committee composed of: - Chair: Pathologist - Secretary: Chief Medical Technologist / Laboratory Quality Manager - Members: One (1) Medical Technologist representative and one (1) Nursing/Ward representative
- Operational Structure: Because TCH operates with a lean six-person laboratory staff, the Chief Medical Technologist / Laboratory Quality Manager executes day-to-day indicator tracking and coordinates CQI activities under the oversight of the CQI Committee.
- Meetings and Annual Schedule: The CQI Committee shall meet at least quarterly to review quality indicator trends, approve CQI project charters before implementation, and update the annual CQI study schedule (F-QM-003-A-V0). Committee minutes and project charters shall be recorded on F-QM-003-B-V0 and reported to Management Review per Management Review.
5.3 Selection, Planning, and Specification of Quality Indicators
- Selection Criteria and Coverage: Quality indicators shall be objective, quantifiable, and risk-based, covering key pre-examination, examination, post-examination, and support processes (including turnaround time, specimen acceptability, IQC/EQAS performance, critical-value notification, corrected reports, customer satisfaction, and test workload). Before implementing an indicator, the laboratory shall confirm that a reliable data source and measurement methodology exist and shall account for any known limitations of the data source.
- Required Planning Elements: Every approved quality indicator shall be documented in F-QM-003-A-V0 prior to implementation with the following specification fields:
| Specification Element | Requirement |
|---|---|
| 1. Title, Purpose & Objective | Indicator name, process measured, and linked quality objective. |
| 2. Methodology & Formula | Data source (e.g., TCH Excel LIS, rejection log), numerator, denominator, calculation formula, and unit of measure (%, minutes, or count). |
| 3. Target / Threshold & Interpretation | Measurable performance target or limit, basis for the threshold, and criteria for assessing trends or unacceptable variation. |
| 4. Collection & Reporting Frequency | Schedule for data capture, responsible role (Owner), reporting interval, and trend display format (table or graph). |
