IG-21 Amended Reports — LAB-QM-POST-001

1 · Before release

The MT performs and verifies the results.
Verify patient ID and data integrity.
The Pathologist authorizes release.
An aide hand-carries the report to the requesting unit.

2 · Error found after release

Notify the physician within 30 minutes (F-QM-POST-001-A-V0).
Recall and void the incorrect distributed copies.
Regenerate the report marked AMENDED, with the reason stated.

3 · Close the loop

The amended report is redistributed to the requesting unit.
The physician signs the amended section to acknowledge it.
Both versions are retained — the original and the amended report.
